Back I have fixed it already, basically ran e2fsck on the ejected disk /dev/sdh, it reported bad superblock on both the main and backup superblock, re-partitioned the drive and rewrote the superblocks using fdisk then used the mdadm command with the -a option to add the disk back into the array.
