We had the same problem here, our spam and virus filter would cause each SMTP
connection to take up to a minute. This is obviously unacceptable so our
solution was to set up another mail server at our main site:

No.1 is used by our customers, doesn't run any filtering, is NOT listed as an
MX for any domains and relays everything to our second mail server. When
sending email through this server, customers experience no delay at all.

No.2 runs all the spam and virus filters, it is listed as primary MX for all
the domains we are hosting, and is responsible for delivering mail to local
and remote users.

Implementing this using sendmail is left as an exercise for the reader. We
switched to qmail several years ago and we're NOT going back :-)
