Yes...No...Yes respectively.
An ISP I used for several years actually have a script on their end that will ping toward the subscriber and deliver the result. Try going to http://www.airnews.net/members/support.html and clicking the "Run a Reverse Traceroute" button. If the site of interest is under your control, you can do something similar. Otherwise, I guess you'll have to be satisfied THAT the echoes get back and let HOW remain a mystery.
