The mke2fs -b4096 process have completed on the 2 RAID sets. I can now see the 2 x 4.0TB ext3 file systems.

Peter, There isn't a RHEL4-AS-U1 channel explicitly declared under RHN. As RHEL4-U1 has been released the main rhel-{arch}-as-4 channels now include all of the updated packages that ship as part of U1.

This indicates that you are running the pre-U1 kernel. U1 is shipped with 2.6.9-11.EL.smp. It's worth checking that 2.6.9-11 is present in the output of rpm -q kernel and if so it seems likely that you simply need a reboot. Check at boot up to see if 2.6.9-11 is (a) present in the grub menu and if it is (b) is it the default.
