Oh, you're using LVM.  It looks suspiciously like a disk configuration
issue here with the LVM volume groups.  The system is trying to locate
the disk labels on VolGroup00 and isn't finding them.  Therefore it
can't format the new filesystems since it doesn't know where they are.

Can you try it WITHOUT using LVM just as a test and see how that goes?
